Insurance
& Payment Policies
If you are Insured:

We
will submit your claim to your insurance company
within ten days of your procedure.

Prior
to surgery, you may be responsible for payment of
your deductible and copayment.

All
accounts are expected to be paid in full within
sixty days. Your assistance in ensuring prompt
payment from your insurance company is greatly
appreciated. Since the insurance contract is an
agreement between you and your insurance company,
any unpaid balance remains the responsibility of the
patient and/or person who signs for financial
responsibility of the account.
Statements
Within
ten days of your procedure, you will receive a letter
from the Surgery Center that summarizes the total
facility fee for your procedure and states that your
insurance company has been billed. To avoid payment
delays, please let us know as soon as possible if this
insurance company information is incorrect.
Statements are mailed monthly to those patients with
balances due. Payment is due upon receipt of the
statement.
Method
of Payment
Payment to the Women's Physician Surgery Center may be made by cash, check
or credit card (VISA, MasterCard or Discover.)
If
Your Surgery is not Covered by Insurance
Total
payment is required on or before the day of surgery, If,
because of financial hardship, full payment is not
possible, please contact our Business Office at (317)
228-1100 as soon as possible to discuss payment
arrangements.
We will do all that we can to help, but
ultimately the responsibility for payment of the account
resides with the patient and/or person who signs for
financial responsibility of the account.
Additional
Fees for Service You May Receive
You
may receive other fees related to your surgery that are
NOT part of the Surgery Center's charges. These may
include fees from the surgeon, anesthesiologist,
pathologist, radiologist and testing performed at other
facilities.
